EGD Upper Enoscopy procedure Dr Srikar Reddy Mapakshi 1/31/2023 performed at UT Health East Texas – Athens location. The EGD is a recommended by Dr. Mapakshi = Gastroenterology Specialist. My primary care provider Dr. McMullen referred me to Dr. Mapakshi to address abdominal pain and acid reflux.
This EGD was the first time I actually had knock-you-out general anesthesia.
All Of The Lovely Details Of EGD Procedure
- On this day, there were multiple patients in the pre-op area. Going by what I was hearing _ sounded like most of the other patients were getting colonoscopy procedures. All of the patients including myself, seem to be part of planned routine camera day.
- The prep for this procedure was fairly simple. An anesthesiologist came to see me and asked me a bunch of questions. They said they were going to use some kind of teeth / mouth guard to protect my teeth.
- The nurse tried multiple times to get an IV started, but she couldn’t get a vein. After she stuck me multiple times in both arms, she finally called another nurse over and he got the IV started.
- They rolled me into the room and I woke up in another room. They didn’t use a mouth guard for my teeth. All of my front teeth felt like they were kicked in.
- I also realized that I peed my pants. My prep didn’t include any kind of gown, and nobody told me that I may need a change of clothes. The nurse offered me a gown after I peed my clothes. I had to choose between riding home with a hospital gown in freezing winter temperatures, or riding home in pee clothes. So I wore my pee pants home after my EGD. The nurse gave me a pee pad for my car seat.

Extra Attention With These Medical Records
Cardiovascular: 11/8/2022 was the first instance of noted Tachycardia. 12/14/2022 notes say normal, 12/22/2022 No diaphoresis noted
Neurological: On 2/8/2022 McMullen noted that I showed no signs of neurological issues. Negative for dizziness, tremors, seizures, weakness, light-headedness, numbness and headaches. On 3/8/2022 The notes for neurological only say “she is alert”. There is no mention of the other neurological stuff previously mentioned. 3/23/2022 McMullen noted negative for dizziness, weakness, light-headedness and numbness. 4/22/2022 visit just notes “she is alert” for neurological. 11/8/2022 everything for neurological was noted as normal. 12/14/2022 records just say she is alert, 12/22/2022 No somnolence was noted 1/31/23 no TIA or seizures
Psychiatric /Behavioral: 11/8/2022 notes were made that My mood was normal ect. But that I was positive for “sleep disturbance” 12/14/2022 records say normal 12/22/22 Notes say cooperative, Negative for agitation and confusion
Constitutional: On 2/8/2022 McMullen noted “she is obese. She is not ill-appearing, toxic-appearing or diaphoretic.” I believe the term “Obese” that McMullen was incorrect. I believe the correct term for me would be “Class III obesity” because my BMI at the time was over 40. On 3/8/2022 McMullen noted “she is obese”. Again, I do not agree we the label of “Obese” I believe the correct term to label me should be Class lll obesity since my BMI is over 40. 3/23/2022 McMullen labeled me as “Obese” again. 4/22/2022 again just noted as “obese” this is 2 days after the radiologist noted “fatty liver” on my sonogram. 11/8/2022 Dr. McMullen diagnosed me with Class 3 severe obesity. 12/14/2022 records say normal. 12/22/2022 say normal with Morbid Obesity noted.
Gastrointestinal / Abdominal: On 3/8/2022 Dr. McMullen did not comment on gastrointestinal issues or left CVA” tenderness as previously mention on the 2/8/2022 records. 3/23/2022 McMullen noted abdominal pain, left CVA tenderness in the area for gastrointestinal symptoms. 4/22/2022 nothing noted about gastrointestinal symptoms. 11/8/2022 Abdominal pain, abdominal tenderness, and epigastric tenderness were noted 12/14/2022 Notes say that abdomen is soft with abdominal tenderness (epigastric). No guarding or rebound. 12/22/22 say negative for gastrointestinal symptoms.
Anesthesia For EGD Procedure
- I found that anesthesia for my EGD was given to me by APRN-CRNA / RN Pamela Gail Frederick. I noticed that Mrs. Frederick was not a doctor. Google says that RNs can give patients anesthesia to patients under supervision of a doctor.
- So I tried to get information about the supervising doctor. MyChart has the anesthesia doctor listed as “D Wilson at UT Health Athens Gastroenterology Lab“. I tried to find D. Wilson in the Texas Medical Board website and I couldn’t find any Dr. Wilson fit in with my treatment. The closest Dr. Wilson I found was a new doctor, that wasnt a doctor in 2023.
- Its possible that the Texas Medical Board cleared out information that I just couldn’t see 3 years after this procedure. Just mentioning it because its weird.

I was given a urine pregnancy test. Came back as NEGATIVE. I have had my tubes tied since 2004 so its impossible for me to be pregnant, but they test my pee multiple times anyway.

More About My Experience with This EGD Procedure
I want to mention this. All though this medical report is complete. I wasn’t given this report and nobody from the gastro clinic contacted me to schedule a follow-up with Dr. Mapakshi.
I believe I received a text message on my cell saying that I had new test results in MyChart. So I logged in and seen that they were negative and that I had mild gastritis.
I didnt know that Dr. Mapakshi said not to take NSAIDs, nobody told me this. I was also unaware that the doctor wanted to see me again.
It is now 2026 and 3 years after this day – I see what happened and why I never got all of the doctors information. My anesthesia for the EGD started at 10:02 AM. Dr. Mapakshi was already entering information in my medical records by 9:36. There is a Result Encounter Notes by Norse Tracey W. at exactly 10:40 AM saying “Notified PT.” I was obviously still out of my head at 10:40 AM, so I don’t know what the heck she said that she notified me about.
My medical records also say that a Nurse Tracey W called me on 1/30/23 to confirm the EGD appointment.

- The medical bill said that My Ambetter Health insurance policy covered $7,272.39. My total paid out of pocket was $357.57. The bill also said that a refund was issued to my debt card in the amount of $72.01 on 2/22/23.
- I looked in my bank statements to see if I could verify that refund, but I didn’t have estatements past 24 months. So I am unable to verify that refund.

On 1/31/23 I Had Been Taking Pantoprazole NONSTOP For:
- 11 months 23 days = 9 MONTHS 23 DAYS longer that recommended treatment time
- or 51 weeks 0 days = 43 WEEKS longer than the recommended 8 WEEK treatment cycle
- or 357 days = 301 DAYS longer than the recommended 56 Day Treatment time
Food And Drug Administration recommends 8 week treatment cycles to avoid adverse side-effects. My primary care provider chose to ignore the FDA warnings in regards to side-effects for long-term use of Pantoprazole.
