Nurse Carolyn J UT Health Gastroenterology in Athens telephone visit 5/31/2022. This phone visit is part of my medical records. There is a bit of confusion, because the notes of this call don’t match with my other medical records.
In the visit notes it says that the sonogram of my pelvis was declined by my insurance. She said the insurance company requested proof that the sonogram was needed. Then goes on to note that she faxed over the results of something but wasn’t specific as to what results.
The notes with Dr. Takata mentioned a CT of abdomen, but no mention of a sonogram of the pelvis. Also, the office visit with Dr. Takata did not have any kind of test performed – not sure as to what results were faxed.

- But the new prescription says it was started by Nurse Kaitlynn B, not Carolyn J.
Continued Prescription Medications
- pantoprazole (PROTONIX) 40 mg EC tablet, Take 1 tablet (40 mg total) by mouth 1 (one) time each day., Disp: 90 tablet, Rfl: 3
- clotrimazole (LOTRIMIN) 1 % cream; Apply topically 2 (two) times per day.
Stopped Prescription Medications
- doxycycline (VIBRAMYCIN) 100 mg capsule; Take 1 capsule (100 mg total) by mouth 1 (one) time each day. SUN protection

Billing For This Telephone Visit:
I did not find any kind of bill linked to Nurse Carolyn. I also did not find any bills for the test that she claims to have sent to my insurance company. So if there were any test results sent to anyone, then they were not billed through MyChart.
This page was written on 11/25/2025 more than 3 years since this phone visit happened. The visit notes and sonogram of pelvis do not fit in with my previous medical records. I believe that my medical records are incomplete and possibly incorrect.
If some results were sent to my insurance company for a sonogram of my pelvis – those results are not in my medical records. Nowhere to be found in my UT Health East Texas medical history.
On This Day, I Had Been Taking Pantoprazole Non-Stop For:
- 3 months 23 days
- or 16 weeks 0 days = 8 weeks longer or DOUBLE THE RECOMMENDED TREATMENT CYCLE
- or 112 days
The FDA recommends a 8 week treatment cycle. Doctors are supposed to monitor patients conditions with the goal to try to get them off the medication as quickly as medically possible. The reason FDA makes these recommendations is because of the well documented possible severe long-term side effects.
I know Nurse Carolyn mentions having test results sent to the insurance company to get an pelvis sonogram – but there were no test performed. I’m not really sure what test results she was referring to. There was also no sonogram of my pelvis ordered from any doctor. Dr Takata ordered a CT of my abdomen.
