Dr. J Takata UT Health Gastroenterology Athens telephone visit on 6/2/2022. This telephone visit says that the call was with Nurse Carolyn J, but Dr. Takata is on top of the record. – so Nurse Carolyn was calling on Dr Takata’s business.
This telephone visit notes says that my health insurance denied authorization for a CT of my pelvis. It also says that Dr Takata would need to order another CT order with only the CT of my abdomen without the CT of my pelvis. The Notes say that Paula with UT Health East Texas scheduling is the one that said new CT orders would need to be made.
There is no mention of a sonogram of my pelvis in this telephone visit. Or any kind of sent test results mentioned the the Telephone visit 5/31/2022.
I do know that this visit happened in 6/02/2022. In my medical records – I do not see further test results in 2022.

- pantoprazole (PROTONIX) 40 mg EC tablet, Take 1 tablet (40 mg total) by mouth 1 (one) time each day.
- doxycycline (VIBRAMYCIN) 100 mg capsule; Take 1 capsule (100 mg total) by mouth 1 (one) time each day. SUN protection
- clotrimazole (LOTRIMIN) 1 % cream; Apply topically 2 (two) times per day.
- sulfamethoxazole-trimethoprim 800-160 mg per tabletCommonly known as: Bactrim, Take 1 tablet by mouth 1 (one) time each day. To prevent boils. Stop doxycycline
The Doxycycline was still listed as a current medication with the “stop doxycycline” as noted above. Im not sure if I continued to take the doxycycline after starting the other medication.
So in this particular medical record the medications are INACCURATE / WRONG.
UT Health East Texas / Ardent Health were made aware of these issues. To my knowledge no changes have been made to correct their medication accuracy problems.

I did not see any medical bills for this telephone visit in MyChart. I’m unable to check to see if any claims were made through my Ambetter Health Insurance, since took much time has passed.
How Much Pantoprazole Have I taken So Far On This Date?

FDA recommended medication treatment cycle for Pantoprazole is 8 weeks. So up to this date, I had already been taking the Pantoprazole for more than double the recommended treatment time. All of my medical records up to this date have been posted here. There is NO MENTION of the Pantoprazole from any doctor or nurse since Dr. McMullen prescribed to me 16 weeks and 2 day prior to this day.
