Lets explore Texas Medical Board feeling of “Lack Of Supporting Evidence” and saying the matter is closed. I think this is a great time to look at some of the evidence I sent to the Texas Medical Board to prove my complaint is valid.

There are a lot of documents I sent in to the Texas Medical Board, but I’m not going to add every document to this article. I will add more document later in other articles, but I really want to put all of the attention into this image.

I have actual decided to just publish 1 specific document because just this single simple image should be plenty of supportive evidence to have an investigation done.

Supporting Evidence For Medical Board Complaint

This specific document was sent in as a supporting document for my complaint about Dr. McMullen changing my medical records to hide medical malpractice. I sent the document through email to appeals@tmb.texas.gov on January 6, 2026.

This document is the lower portion of my medical record for a clinic visit with Dr. McMullen on April 3,2025. I submitted this document to prove that medical records have been manipulated / changed / altered illegally.

HERE IT IS…

So this document was for a clinic visit on 4/3/25. During this clinic visit I tried to tell McMullen that the medication Phentermine was causing some serious side effect. I told her that I stopped taking the medication on 4/1/25 because I thought it was hurting me.

McMullen disagreed with Phentermine as being the cause for the side-effects. I ended up just going with what she said, because I’m not a doctor. I trusted her medical care, so I didn’t really push too much on my thoughts of the Phentermine causing a reaction. This difference of opinion really isn’t important to this article.

The dates in this information is important so I have highlighted them.

It is important to know and understand that as of 4/1/25 I had not taken anymore Phentermine. McMullen was aware of how I felt and that I was no longer taking the medication during the 4/3/25 clinic appointment. There were also no new medications ordered on 4/3/25. McMullen did order a few blood test on 4/3/25, but the results were posted next day.

This appointment was on 4/3/25 and my next appointment with McMullen wasn’t until 4/25/25. So there is a 3 week gap between clinic visits.

So there should not have been any kind of changes / alterations / or manipulations of my medical records from 4/3/25. Why would McMullen need to change my medical records during this 3 week gap between the clinic visits?

As you can see in the image below, McMullen made changes to my 4/3/2025 medical record on 4/17/25. She had ZERO reasons to even think about my medical records on 4/17/25.

Texas laws say that if a doctor makes a medical record change like this, they are supposed to note the reason for whatever changes were made. The are also supposed to be completely open and transparent about any changes and not remove any of the original record.

Texas Medical Boards Decision To Close Due To Lack Of Supporting Evidence

While it is a bit disturbing that the Texas Medical Board wouldn’t see an issue with this image. While this particular document wasn’t the most important I sent to support my complaint – it does have significant value.

This image proves that when I told them that my medical records were changed – my complaint was TRUE. The dates are right there plan as day, very easy clear proof of manipulating medical records.

At the very least, this should have been enough supporting evidence for an investigation with just this single image.

The Texas Medical Boards decision based on lack of supporting evidence is disturbing, because apparently these types of medical record changes are OK. So if McMullen can do it without an issue, other doctors can do the same without any issues.

This article was created to share my disbelief and dissatisfaction with the Texas Medical Board. This image originally came from the MyChart medical records from the UT Health East Texas Hospitals. I took a screenshot of the medical record for the purpose of proving a TMB violations and I sent them to TMB with a valid complaint.

The image is only of a small bottom portion of the actual medical record for 4/3/25. I used this image because both dates are visible to prove manipulation. And of course, these are my personal medical records that I can share – I am the patient the records are attached to.

Why This Texas Medical Board Information In Important

I will be publishing more of the documents I sent in to the Texas Medical Board. I don’t feel like there was a lack of supporting evidence. I thought that if anything – there would be too much supporting evidence.

I think its important to share this information publicly, to help other patients with their Medical Board Complaints. Texas Medical Board doesn’t like this image as supporting evidence – so others should know what documents Medical Board wants to see.
