This telephone visit with Kaitlynn B on 5/31/22 was part of a bit of confusion. Apparently earlier in the day, I had a separate telephone visit with Nurse Carolyn from Dr. Takata’s gastro clinic.

- For the “Telephone Visit” with Kaitlynn B – the notes say that I stopped by the clinic in person, not by phone. Says that I went to the clinic asking for help with medications, because I started getting more heartburn after starting Doxycyline. Says that she sent a message to McMullen at 1:23PM that day.
- Then at 5:54 pm that day McMullen says to stop taking Doxycyline and start taking Bactrim. She also noted that sent, so that meant she sent the RX to the pharmacy.
- The following day on 6/1/22 at 9:46 AM, Kaitlynn B notes that she notified me about my medication.
On the documents from the telephone visit with Nurse Carolyn, it says that Nurse Kaitlynn B changed my medications.
There is a note in bold letters that says “Accurate as of May 31, 2022 11:59 PM”. So it looks like MyChart combined the 2 separate issues in documents, but still with 2 separate After visit Summary and Notes From Care Team.
New Prescription Medications
- sulfamethoxazole-trimethoprim 800-160 mg per tabletCommonly known as: Bactrim, Take 1 tablet by mouth 1 (one) time each day. To prevent boils. Stop doxycycline
- But the new prescription says it was started by Nurse Kaitlynn B, not Carolyn J.
Continued Prescription Medications
- pantoprazole (PROTONIX) 40 mg EC tablet, Take 1 tablet (40 mg total) by mouth 1 (one) time each day., Disp: 90 tablet, Rfl: 3
- clotrimazole (LOTRIMIN) 1 % cream; Apply topically 2 (two) times per day.
Stopped Prescription Medications
- doxycycline (VIBRAMYCIN) 100 mg capsule; Take 1 capsule (100 mg total) by mouth 1 (one) time each day. SUN protection

Time On Pantoprazole For This Date:
On 5/31/22, I had taken Pantoprazole non-stop for DOUBLE the recommended FDA Protonix treatment time without being tested for possible side effects. FDA recommends a 8 week medication period and this day marks 16 weeks taking the drug.
